Complete two Nursing Diagnosis statements for the family.
A properly formatted family health nursing diagnosis addresses either preschool-age children, single mothers, or pregnant women.
This is similar to what you would do for an individual patient and what you did for the community, but now our “patient” is the family.
The family diagnosis statement should be written as follows:
______________(whatever family nursing problem you identified) related to___________ (here you put the cause of the problem also known as etiology) as evidenced by__________________ (Here, put the evidence–the signs and symptoms. Remember, if it is a “Risk For” nursing problem, there is no evidence as the problem hasn’t happened yet and we are trying to prevent it.)
Now complete ONE Community Nursing Diagnosis statement which should be formatted as follows:

Expected outcome: Complete one SMART goal (Specific, Measurable, Attainable, Relevant, and Time-Bound), which is opposite the problem for each individual Family diagnosis statement. You will need two goals. You do not need a goal for the community diagnosis statement. Start your goal with “Family will….”
Include two nursing interventions for EACH goal AND rationales for each intervention.
Describe how the nurse will evaluate if the goals have been met.
The single mother’s name is Candace Conway, she is 26. Has two children and is pregnant. Her children have two different father’s. She lives in a two bedroom above a laundromat. She moved in to help her grandmother but she passed 6 months ago. She left her a furnished apt but still has to pay the rent which is $1250 a month and includes water and electricity. her son Braydon has some sort of disability where his speech is delayed. Bcs he has been bullied he has developed a bad temper. She works as a cashier at the grocery store. It only pays $12phr and she can only work four days a week for four hours since the boss doesn’t allow for more hours.
Since her grandma died, her landlor has been watching the kids for $40 aweek. Some nights she leaves Brayden in charge. She doesn’t do it often since neighbors have been watching.
She isn’t getting any prenatal care. Not planning on breastfeeding and the child’s father comes and goes. Very unstable.
Her bf Jessi uses meth. He barely pays childsupport and doesn’t spend time with the kids. She doesn’t have a plan for babysitting once she returns to work. She doesn’t want to leave the kids with Jessi bcs he has a bad temper. He steels from her and she is aftraid she will be kicked out soon due to lack of rent payment.
She smokes pot with him and drinks alcohol despite her being pregnant. She also is down to a half a pack of cigarettes a day.
