Ms. J is a 42-year-old Caucasian female who presented to the ED with complaints of a throbbing headache that started yesterday and is increasing in severity. She denies a history of headaches. Denies changes in vision, dizziness, or trauma. Denies alcohol, illicit drug, or tobacco use. Ms. J ran out of amlodipine for her hypertension but had been taking her levothyroxine every morning, which she takes for hypothyroidism. She reports eating healthy and walking at least 30 minutes every day. The last cervical screening was 6 years ago and showed no abnormal findings. Her last menses was one week ago. On examination: VS 37.0 oral temp; BP 169/89, HR 65, RR 18, SpO2 100% RA with PERRLA, and EOM intact. Negative Brudzinski and Kernig’s sign. Negative pronator drift, HINTS exam, and no focal weakness observed. Headaches are not provoked by touching the face. The headache does not prevent her from sleeping or walking.
